SEGMENTAL ASYNERGY AS AN EXPRESSION OF AFTERLOAD MISMATCH IN A CASE OF AORTIC STENOSIS

Emergency aortic valve replacement with aorto-coronary bypass surgery was performed on an 82-year-old man because of severe intractable congestive heart failure; mild degree of circumflex and left anterior descending artery lesions were seen and the pressure gradient across the aortic valve was 80 mmHg in spite of the low cardiac output status. Antero-septal akinesis seen by echocardiography normalized after the surgery. Segmental asynergy can be one expression of afterload mismatch due to tight aortic stenosis when concomitant mild ischemic heart disease is present.
